Payment Method

Cauley Counseling Services requires a credit card from ALL clients to be on file in our secure online portal Simple Practice. We accept credit cards, debit cards, health savings accounts (HASA), or flexible spending accounts (FSA) cards (with Visa logo) as payment. If you need to change your card on file or use a different method of payment, please notify me prior to the start of your session.

Insurance

Since I am not contracted with any insurance companies. I do not accept health insurance for services. As an out-of-network provider, I maintain my commitment to confidentiality and allow you to drive the therapeutic process. By seeing an out-of-network provider, you are not confined by the insurance company's parameters regarding mental health.

I can offer a superbill, in which you can submit to your insurance company for reimbursement using your out-of-network benefits. If you have PPO insurance. To find out what your insurance may cover, call your insurance company and ask if you have out-of-network mental health coverage. HMO’s usually do not reimburse for providers who are not in network.

Insurance Coverage Information

To find out what your insurance may cover. Contact your insurance provider by calling the number on the back of your insurance card for the benefits department and ask about your out-of-network mental health coverage. If you need to provide a “CPT code” or “service code” please provide the following codes:

  • Initial Intake/Biopsychosocial Assessment - CPT 90791

  • Individual therapy, outpatient – CPT 90837

  • Couples/Family Therapy, Outpatient - CPT 90847

  • Our office zip code is 76006

  • My credential is LPC

Good-Faith Estimate

Under the law, as a healthcare provider, we will give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services including psychotherapy.

  • You can request a Good Faith Estimate in writing at least 3 business days before your scheduled therapy service or anytime during treatment.

  • It is recommended that you save a copy of your Good Faith Estimate for your records.

The Good Faith Estimate is based on if we met for 50 sessions during a calendar year. If I see you 50 minutes a week for 50 sessions, you would expect to pay $7,500. If you receive a bill that is $400 more than your Good Faith Estimate, you can dispute that bill, as explained in the Good Faith estimate.

Cancellation or No-Show policy

We have a 24-hour cancellation/rescheduling fee policy. To cancel or reschedule for any reason make sure you give us 24 hours’ notice to avoid paying the late cancellation/late reschedule fee. which is the full session rate. If you no-show for your session you will be charged the full fee.
